This month’s release includes enhancements to your online appointment request forms, helping you be more prepared for appointments and providing better patient care.
Appointment Request Forms
Appointment request forms have been given a makeover so you can capture patients' details ahead of their appointments. The new forms are dynamic meaning you’re in control with how you configure them based on your practice needs.
- Patient Type: New or Returning
- Patient Info: This includes first and last name, email, phone number, date of birth, street address and apartment number, city, state, and zip code
- Message for the Office: This is an optional field where patients can add notes for the practice.
- Insurance Info: This includes insurance provider, subscriber first name and last name, member ID, and subscriber date of birth. RevenueWell will run an insurance verification check if the patient and their insurance info exist in the PMS. Practices must have RevenueWell’s Insurance Verification for it to trigger.
- Compliance and Submission: This includes a consent checkbox that must be checked, a CAPTCHA to verify the patient isn’t a bot, and a submit button. Patients can click the submission button once all required fields are complete. Missing fields are highlighted so patients can fill those out.
- Practice Information Sidebar: This includes the practice name, reviews, address, phone number, email, website, and business hours. Information is pulled from your practice settings.
- Appointment Info: This is dependent on how you’ve configured your appointment type in the Scheduling settings. There are two types:
- Simple Appointment Request: This is triggered when the PMS Calendar Availability is turned off. The form asks patients for preferred days/times (Morning, Afternoon, Evening), based on the practice’s availability settings. Multiple preferences can be added using “+ Add another day/time”.
- PMS-Based Scheduling Request: This is triggered when the PMS Calendar Availability is turned on. Patients can choose a specific date and time from available spots pulled from your PMS.
